I always disable VoiceOver speech via three-finger double-tap when using Siri. that way, I don't miss anything she says. Then I reenable speech the same way.
For me, Siri only talks under the following conditions.
I press and hold the home button.
I press the talk button on the screen.
I say Hey Siri followed by a command.
My phone is charging and there are voices in the room, people talking, TV is on etc. and Siri thinks someone is talking to it.
If your Hey Siri is not on, you might want to try a factory restoer using iTunes and don’t install anything for a couple of days and see if the problem goes away. If it still does it and nothing is installed, I would check with Apple.

Is Hey Siri enable and your iPhone plugged in? IF so, Siri might think your are talking to it.
So it’s not a bug, it’s a feature.
