I am using an iPhone 15 Pro running iOS 26.5.2. Every now and then, I experience the strange issue when I am using my phone. It is completely random, so I cannot give you an accurate timeline or what I can do to reproduce this bug.
My phone will make the default system notification sound, but there is literally no notification that I know would have triggered that sound. There is no notification banner at the top of the screen, and there is no notification within the notification center that triggered the sound. My phone just randomly will play the default system notification sound, and it also vibrates.
I am not sure what is going on, as I don't have a focus mode turned on. I have all of the apps set to send notifications to the lock screen, allow as a banner, and appear in the notification center.

Live Activities
Is it possible you're triggering a live activity in one of your apps? Those will appear at the top of your screen and may be visible on the lock screen.
It's also possible that you are in fact getting a notification from an app, but per your notification settings the notification is not actually configured to display on screen or in the notification center.

I think I might have fixed it.
One of my apps said that the notification sounds were turned on, but the notifications were not set to appear on the lock screen or in notification center. Hopefully this fixes the notification issue. I change this app's notification permissions to appear on the lock screen and as a banner.
