The iPhone maintains a memory of each thing you connect. Try initiating some Voice Over speech, and pressing the volume up button repeatedly while connected to your car.

The voiceover volume is always much lower than the normal sound/volume that comes from a radio station or music in our car. I have also experienced the same when I connect my phone to my Bose radio via the auxiliary port. Turning up the voiceover volume has resolved the problem.
