On screen keyboard issues since updating to iOS 26

I don't know if anyone else has experienced this but its driving me mad and I don't know if its something I've done. Since updating to iOS 26 voiceover seems to be reading out the wrong letters when I am using the on screen keyboard. For example I put my finger where the letter A is located on the keyboard and voiceover will read out the letter T. Its very random and the only way I can fix it sometimes is by switching to numbers and then back to letters. It is happening on multiple devices for me and using different voices but thinking it maybe something I've done by accident in settings somehow.

When I tap a letter VO does not read it and makes me wander if I did so, when I check the letter was tap but VO did not read it.

I do not have this issue on my device but I had some small or minor issues when upgraded to iOS 26.
I am "multilingual", so I use both Norwegian and English -- both languages set up as seperate keyboards in settings.
I also have the emoji keyboard.
Of some reason I suddenly had three keyboards/keyboard languages set up after upgrading to 26.
I found it very cumbersome to now switch between the layouts while typing and when inserting emojis, so I went to settings and deleted all my keyboards and added them again.
I then discovered that I no longer need to have seperate keyboards -- I'm not sure if this is new in 26 but I only had to add one keyboard and I could have one more additional language configured for that keyboard.
So, I added Norwegian QWERTY keyboard and then added English to utilize multilingual support on the keyboard.
I double checked that both Norwegian and English was set up as QWERTY layout and then added the emoji keyboard.
When I write text in Norwegian the keyboard auto detects that and spell checking etc is Norwegian and when I write English the keyboard switches to English dictionary.
So, I no longer need to switch between Norwegian and English keyboards.

Ok, I recommend to verify your keyboard settings or just delete all keyboards and add them again.
You could also try to reset VoiceOver settings.
Resetting VoiceOver settings to factory defaults is a brilliant new feature in iOS 26.

It turns out that the culprit is the VoiceOver setting called standard typing! If you go into VoiceOver settings and change keyboard input to Touch typing then the problem disappears if after using touch typing for a few words you change back to standard typing the problem does not reappear immediately but will do so later on. I have been writing to Apple Accessibility and The sending them screen recordings of this problem in action for a week or so now. They have asked me to send diagnostics but their link simply simply hangs up telling me that they are they are trying to connect to Apple Support so that is a fat lot of good. But the problem is genuine I can tell you and I have noticed this morning that it also happens on my iPad Pro M4. To be clear, the problem does not happen all the time and I’m not sure exactly what the pattern is although I have noticed it most in the notes App but this morning I noticed it in the search tab of the Podcasts app on my iPhone 16 pro Max so do you go ahead and report this to Apple because the more people who reported the sooner will get a fixed!
