This is a new one to me, but I am wondering if VoiceOver is getting confused between your BSI settings and your standard touch keyboard settings. Have you tried disabling character and/or word feedback while typing using the standard touch keyboard with VO? If not, try doing this, and then go back to BSI and try typing and see if you get any voice feedback at all?

A possible workaround, but not a permanent fix.
Try going to settings, VoiceOver, Typing, and then navigate by heading for both hardware keyboard, and BSI, setting both of those to — "None".

I noticed this change when iOS18.3 came out. I am annoyed by it because I want typing echo off.
I actually switched the braille table in BSI settings to uncontracted braille to see what happens, and in that case my typing echo settings are honored.
This leads me to wonder if something with braille translation is causing the VO feedback?

I guess we'll have to see how things go with iOS 18.4. 🙁
