Everything. As user KE8UPE has pointed out, it is highly configurable and optimized for VO users. I also really like how you can choose where the tabs are displayed on the iPad, and all four vertical columns are visible. Apart from the customization, I like how CW posts are displayed if you choose to automatically expand them (most clients just exclude the CW text altogether). I also appreciate how Mona displays my filter warnings: VO says the user and then the filter warning. Last but not least, I like how I can apply different filters to my home timeline and lists, so I can group people based on how much of their content I actually want to read.
I have never come across such a truly accessible and feature rich app, the customisations seem to be endless. I really enjoy the tab bar and post action customisations, and well… Everything. It’s definately worth the money in my view but obviously that’s a personal decision, but having tried metatext then toot due to metatext not being actively supported, I am glad that Mona is out there now. The best one out there in my view for both accessibility and customisation by far.
The only issue I have is that I check my notification in my iPhone and sometimes when I go back to my iPad, it shows notifications as unread. They are not cincing. Not nice. I am using pro max.

Great to see the continuing evolution of @MonaApp. The ability to set custom notification sounds not just on an account by account basis, but also for each notification type, is very impressive. And the sounds come from members of the #blind community too. I regularly monitor four mastodon accounts and there are sufficient sounds for everything I want to be notified about to have a unique one.

Version 5.3 of this as well as the Spring app have been released as of today. Both updates include new sounds.
