VoiceOver in Login Screen

Since one of the last updates my Macbook with latest os does not switch on VoiceOver automatically on login screen. When I turn on to do so in System Preferences Users and login options, the setting is not permanent. It is set to offf after next login. In addition, on login VoiceOver hangs. I habe to deactivate and re-activate it by pressing CMD+F5 twice. VO hangs in some situations while using BlueTooth with my headphones. In this case I also press twice CMD+F5. Has anyone an idea how to repair this? Best regards , Guenther
