If you want to use a self-hosted service, Reeder should work with services which use the Fever or the Google Reader API. Or just standalone RSS (without syncing) if you don't want to use any service.

Barely anything is accessible with VoiceOver - most UI elements are either not recognized at all or are simply reported as unlabeled buttons or meaningless “dings”.
Based upon where things are now and having read about the developers lack of communication with users, I don't see this becoming accessible any time soon. Avoid and save your $10!
