Maybe it has been exposed to liquid of some sort My iPhone XS Max has acted like that, and got notification that a liquid has been detected in the lightening port, I gave it an hour or so and the speaker is back to normal.

When I received my brand new iPhone XS, I had no problems with speaker distortion. But within a couple of weeks, the speaker began to buzz. This occurred whether VO was speaking, music or books were playing, or a phone call was in progress. Contacted Apple, sent the device in for repair, they replaced it instead. Same problem when I took it out of the box. I sent the second device in again. Same thing. They replaced it and when I got it, the phone started buzzing within a couple of days. Very frustrated. I am using the third iPhone XS but still deal with the buzz. Phone calls no longer buzz but I still have the interpherence elsewhere. I have no idea what the problem is, but since two replacements didn't fix it, I just live with it now. In my opinion, I shouldn't have to pay over $1,000 for an iPhone and have this problem. Prior to my experiences with the speaker buzz, I have had no problems with any iPhone dating back to iPhone 4, my first device.
