For many years and iOS versions I’ve noticed that my phone will occasionally not fall asleep though the auto lock is set to 3 minutes and I would powered off and on the phone or do a hard reset to start things working again properly.
Just recently with the release of iOS 15 I noticed that when this happens if I triple tap the home button to turn voiceover off then triple tap again to put VoiceOver on low and behold the sleeping with auto lock works perfectly until the next time!

Things to try
When I've had this problem in the past, I've found that cleaning the screen often helps.
Another thing to try is a soft reset. We used to be able to do this by holding the home and sleep buttons for 15 seconds, but I think it varies by model now. With my iPhone SE, holding just the sleep button for 15 seconds seems to do the trick.

I also find this problem. I'll get a notification and then find the VO focus goes to the close. The result is the phone announces the time until I manually put the phone to sleep. Very annoying.
