•Open VoiceOver Utility.
•Navigate to Speech>voices>Customized language list.
•Interact and then press Edit.
•In the list, make sure the default voice is set to Samantha and then scroll to additional voice settings, expand this option and interact and make sure all the voices are set to default voice.
Something very important here and based on my experience, make sure your Mac is not using low energy mode, the quality of the voices and the stability of VoiceOver is greatly affected with this mode is active.

At least this checkbox exists in iPhone.
