So far I have only used touch ID phones. While playing around with an iPhone 12, I noticed an issue while turning Voiceover off and on with triple click side button. While VO does get turned on, the device also switches to power off mode, giving me the options to power off, medical ID etc. What ads to the trouble is that, after I hit cancel, the phone asks for passcode. So if one has to turn off VO and on at the middle of something, this proves really annoying. Somehow the triple click is being interpreted as a power off command?
I have IOS16 on this device, but I don't know whether this is IOS16 specific, as I have not used a face ID device before.
Additionally, I am also noticing that two finger swipe gesture is misinterpreted while using the device with one hand. It works as expected only when I support the device with another hand.
These are issues apart from many hit or miss situation while unlocking the device with face ID. Accessing the Status Bar also appear not as consistent as in touch Id devices.
Any input would be of great help!

Could the cause of the problem be the case used?

The side button areas of some cases can be extremely hard.
I have come across such products in my case reviews a few times.

Voiceover and side button
I have been using iPhones since the 3G
I have had no problems at all turning voiceover on with the side button
It’s three quick clicks
If you hold it down for too long I can see it going into shut down mode
But I’ve never had that issue that you’re describing
You need to click it quickly.
It doesn't need to be held down. I've been using the side button for Voiceover for a few months now and I've never had that problem.
Clarifications
Thanks all for your helpful inputs. Actually the device switches to power off mode after I turn Voiceover off. I am not holding down the button, only triple clicking it. Case also seems not to be the issue, as this is happening even without it. I also tried changing the side button speed in accesibility touch settings but that also didn't fix the problem. The button otherwise is not misbehaving in any other way.
Resolved the issue
This is how I got the problem resolved, in case it may help someone:
1. Turned on triple click side button for Emergency SOS. It prompted me that accessibility shortcut will be disabled. I went ahead and confirmed.
2. Turned the same option back off.
3. Turned back on the triple click option back on for Voiceover in Accessibility setting's
