VoiceOver feedback missing while editing text

I have encountered problems when editing text using voiceover in mail, messages, notes, etc. if I want to erase say the last three letters of a word if I've put the cursor at the end of the word and then tap delete, it does not tell me that letter has just been erased, but if I tap delete again, VoiceOver will tell me the name of the second letter from the end and also works for subsequent deletes it's just the first letter that is deleted. It's not told that it's been done.
Also if I want to add text and I move the cursor to the end of a word in a sentence and then tap the space button invariably I get no audio feedback that the space has been added. I have to go into the rotor, go to characters to see whether a space has been entered. This problem is on my iPhone SE3 and also my iPad mini 5 and is a common though intermittent problem and it's driving me crazy

When waking up my phone, I noticed that there is now a warning message that VoiceOver is on and how to turn it off which comes up as a banner over the top of the status bar for a few seconds.
This happens not only when you wake up the phone, but also on other instances of using the phone and it's very annoying. Does anyone know how to turn this message off? a

Not seeing either of these issues. I’m not saying it isn’t happening I just can’t reproduce it. However, let’s start to drill down on this.

list of 3 items
1. For the person who is getting the following: “VoiceOver is on,” and how to turn it off—what model of phone are you using?
2. What voice are you using?
3. Change your voice to something like Eloquence does it still happen?

I believe this is just a new feature of iOS 26, or at least 26.2. All this does, is to let a sighted person know that they have enabled an accessibility setting, and I believe it shows them how to turn it off again. If you've ever been on YouTube, you can find a plethora of videos asking how to turn off the speech on their iPhone, and now Apple has made it a little easier for sighted people who accidentally turned the feature on, to turn it off.

In answer to your questions item one is a new feature in iOS 26, unfortunately.
Item 2 the voice I was using with Serena premium, which is a UK voice . I have the same problem on my iPad mini 5,
I found that changing the voice made no difference to having the problem.
I should mention that it is just the very first time you double tap on delete that you don't hear any feedback, on subsequent double taps of delete you will hear the letter being deleted. This is also true when double tapping the space Bar. The first time you hear nothing but subsequently every time you double tap you do hear the word space.
It is an intermittent problem.
Apparently Apple has had other complaints from people concerning the same problem so this is not unknown to them.
