I have set this to headphones and car and also tried it with the setting to headphones only. It reflects the same setting under Phone and Siri and the Facetime setting for this. However, VoiceOver still announces the caller name when not connected to Headphone OR Car. Is this setting broken?
P.S. This issue is as old as with IOS 18 it's just getting on my nerves now.

This feature is for Siri telling you who calls, not Voiceover. And yes, this is only working when Voiceover is off. To stop Voiceover announcing your calls there is as far as I know no way to stop this.

I believe muting the ringer will stop VO from announcing incoming calls.

If you still want to hear the phone ring, you could use a voiceover gesture to mute voiceover. I use a three finger triple tap to mute voiceover if a call comes in, you only hear the ringer not who is calling then I just use the magic tap gesture to answer like normal.

From all the responses on this post, I conclude that the issue has not been resolved. Although I appreciate the work around to mute the ringer, this is not the intention and solution I seek. I am reporting this to the apple accessibility team for further investigation. Thank you for your support!I
