Occasionally, when I do a double tap on a button to activate it with Voice Over, it will make the VO double tap sound but nothing will happen. If I go to another app and switch back to the current app, it fixes the problem. Does anyone know what may be causing this and how to stop it from happening? I believe it started about 6 months to a year ago. I'm not sure if it happened with IOS 11 or not. It isn't a big deal but it is mildly annoying. I am using an iPhone 6S, running IOS 12.3.

Double Tap Problem With VoiceOver in IOS
This behavior happens to me as well. Some times it occurs with trying to open emails or messages as well. The only way I can fix the problem is by turning VoiceOver off and then back on again or powering off the phone and powering it back on. it is definitely an annoyance, but I have no idea what could be causing it to happen intermittently.
same here
I'm running into this problem as well now and then... its for sure annoying. We should send this thread to apple accessibility. The only problem is... This happens in 1 out of 10 cases, so its not easily reproducible.
Workaround
Hello,
I have discovered that this issue occurs when you go to control center and toggle something there. The definite fix is to go to notification center and press home button. I told apple accessibility about this but they told me they cannot reproduce.
Hth.
dubble tap problem with voice over
yess, I have same problem, usualy insite the app, IE youtube app.
I have the same issue here
I am running an iPhone 6 with iOS 12.3.1 and I have informed Apple about the double tapping issue also. They just say to turn voice over off then back on and then restart the phone. That's like putting a band aid over a wound. It doesn't solve the problem.
