When editing Safari bookmarks, the bookmark names and reorder icon label are incorrectly announced with "remove" prepended.

Select and play a video in the Photos app.
Place VoiceOver focus on the "Video playback control".
VoiceOver announces the current state as "Paused", although the video is playing.
When you are playing music in the Music app, and you open the mini player to full screen, or the Now Playing screen if you wish to call it that, VoiceOver is speaking the Pause button as Play button.
Enable VoiceOver and select the Alex US English voice.
Invoke Siri by pressing and holding the side button or home button, or with the Hey Siri command.
You will hear the Siri chime, but before you can speak your command, VoiceOver will say "Possible text, followed by the time".
This does not happen 100% of the time, but most of the time.
Only observed when using the Alex VoiceOver voice, but of course it is possible that there are other affected voices.
When using conversation view, aka message threads in Mail, there are issues when you swipe to a new mail in a thread.
If there were four emails in the thread for example, and you receive a fifth. When you open the thread, you can use the up and down swipes in the messages rotor to go through messages 1 of 5, 2 of 5, 3 of 5 and 4 of 5.
However when you swipe down again, instead of hearing 5 of 5, VoiceOver goes silent.
You may find the message by tapping the middle of the screen, but navigation back up through the thread can then become problematic.
In Find My, after selecting the “Devices” tab and double-tapping a device, there is sometimes a delay between touching or swiping the screen and VoiceOver speaking the name of the focused element.
By defaul, there are two shortcut action buttons on the lock screen, Torch and Camera.
VoiceOver now speaks these as "Torch, off, button", and "Camera, off, Button".
Even if you change their function, as permitted from iOS 18, the "Off" part of the label remains.
Whatever the use of the button chosen, the "Off" part of the label is superfluous.
In iOS 14, VoiceOver may 'lag' or feel less responsive than it was in iOS 13. The presentation of this behavior is somewhat inconsistent, and we suspect that the severity of the issue is partly dependent upon the age of the device and is also somewhat subjective. Our experience is that adding a secondary language to the rotor, even if it is the same language and voice as you are using, and then switching to that secondary language improves VoiceOver's responsiveness.
On occasion, VoiceOver becomes unresponsive after ending a call in both the Phone and Facetime apps; this issue sometimes necessitates that the user must perform a force restart to restart the device.
On occasion it may be possible to return the device to a responsive state by using the Accessibility Shortcut to turn VoiceOver off and back on again. Note that this may take 2-3 attempts.
The presentation of this behavior is inconsistent; some members of our team report experiencing this issue several times per day, while others have not experienced it at all.
VoiceOver may not always automatically announce the time when you press the Power button to wake your device.
This problem has been seen in previous versions of iOS, appeared to have been fixed, but has returned once more in 9.1.
When opening the Control Center, VoiceOver does not tell the user that Control Center is opened with a spoken announcement; this is in contrast to other system areas such as the Notification Center, where VoiceOver provides feedback once the gesture is performed. When the Control Center gesture is performed, there is no spoken feedback, and VoiceOver focus is usually (but not always) placed on the first item in Control Center.
VoiceOver is not consistent or reliable in announcing the badges that are displayed on Home Screen icons to alert you of new items (for example, unread email messages).
This behavior has been noticed in particular with the Mail and Messages apps, but also on numerous other apps.
On occasions, a hard restart of your device may temporarily resolve this problem.
This behavior appears to not affect all users, and can vary between users in regard to which app icons will be affected.
This behavior itself is not new to iOS in iOS 11, but we have detected what we believe to be a slight increase in the frequency of voiceOver becoming unresponsive and needing to be restarted to resume normal working behavior. In particular, it seems more common that spoken output from VoiceOver will stop, but the other VoiceOver sound effects continue.
In certain situations, VoiceOver will randomly speak the contents of previous notifications which do not appear onscreen visually. This issue does not occur consistently, and the notifications are no longer spoken once speech is stopped or focus is moved to another element.
Update: since posting this bug here, others have reported also seeing this behavior with voices other than Alex.
Original Report: This bug appears to be dependent upon a very specific use case and has an effective workaround.
If you are using Alex as your default TTS voice, you may find that VoiceOver will switch to use another voice whenever it needs to announce punctuation.
When encountering a precipitation chart in Weather, there is a delay between swiping left or right and hearing VoiceOver speak the contents of the focused element. In some cases, after swiping several times, VoiceOver focus may move to the "Show map" button near the bottom of the screen.
On occasions, VoiceOver will not announce that an email folder is empty when you tap or swipe onto the area of the screen where any email messages would be listed. Instead, the only audible feedback is a muted ‘clunk’ tone. The behavior has been seen in both smart folders and regular folders.
When composing a new iMessage, VoiceOver may not provide feedback on which camera is active if you choose to take a photo or video to add to the message.
If you tap and hold on the button to Attach Media (supposedly the quick option to access this feature), VoiceOver does not announce whether the front or back camera is active.
When entering text using Braille Screen Input and input mode is set to Contracted, VoiceOver does not echo the characters typed—even if typing feedback is set to echo characters. This creates a situation in which the user would have no way of knowing what dots were entered until the word has been translated, thereby making Contracted braille input less efficient because there is a greater possibility for mistakes to be made if the user has no feedback as to what is being typed.
In iOS 9 on an iPad, the Command key on a bluetooth keyboard shows all possible keyboard shortcuts. However, this feature appears to not work with VoiceOver, as no additional information or feedback is given when the Command key is pressed.
